Fast Truck Repair. and its preferred service providers serve as emergency roadside service providers with the purpose of providing repair services to various commercial equipment. Fast Truck Repair utilizes an aggregate network of third- party vendors, and will employ such vendors to diagnose, triage, and/or repair the specified customer disablement. Please understand that not all vehicles can be repaired via roadside service and may need to be taken to a shop for further diagnosis. In this instance, you may still be responsible for the costs associated with our service and the services of the vendor dispatched to assist on the breakdown, including time and mileage.
A down payment is required to initiate service, a $250 minimum. This payment will be applied to the total cost of the service event. All sales are final. In the event Fast Truck Repair cannot locate, access, or you are not with the equipment, a dry run fee may be assessed. In the event the equipment has moved or is not at the disablement site, a GOA, or Gone on Arrival, fee may be assessed.
Accepting these terms gives Fast Truck Repair permission to debit your account for the agreed upon services according to the terms outlined on this Work Order. This payment authorization is for the goods/services described herein. By accepting these terms, you certify that you are an authorized user of the payment method provided and that you agree to all charges related to this service event. Please note that if using a credit card as payment, the credit card must bear the name of your company, or if a personal credit card, must match the name registered with the credit card company. In the event Fast Truck Repair cannot provide service, or a vendor cannot be located, with funds already secured, Fast Truck Repair will initiate a refund of any monies collected.
I hereby authorize Fast Truck Repair, it’s employees or preferred service providers, to arrange for performance of repair work as described in a written or verbal estimate, covering all Labor, Parts, Materials, and other related charges. I agree that Fast Truck Repair, or preferred service provider, is not responsible for loss or damage to the equipment, or articles left in the equipment, in case of theft, fire, or any other cause beyond their control. I agree that Fast Truck Repair, or preferred service provider, is not responsible for delays caused by availability of parts or any parts delays. I agree that Fast Truck Repair, or preferred service provider, is not responsible for delays incurred due to weather, traffic, or police intervention.
I grant Fast Truck Repair's preferred service providers permission to operate my equipment for the purpose of testing and/or inspection. I understand that, if any closer analysis finds additional labor, parts or materials are necessary to complete the repair, I will be contacted for authorization. I acknowledge that this work authorization approved supplemental estimates. I agree that if I should halt repairs for any reason, I will be responsible for the cost of any and all repairs completed to that point, as well as the cost of the parts which are not returnable, or restocking fees charged to Fast Truck Repair, or preferred service providers, if I choose not to purchase said parts outright. I am entitled to retain any parts I pay for that are not returnable to their vendors.
